These forms are provided online as a courtesy for coaches although representation at the training is a requirement for participation in this years events.
Parental Consent Form - one required for every participant
Day Volunteer Form - one required for each volunteer
AwanaGames Team Roster - must be turned it day of event
Sparks-a-Rama Team Roster - must be turned in day of event
Bible Quiz Team Roster - Information may be emailed as well
