2.  Important Infomation your registration is not complete without this form

Each church must complete, sign, and fax the following form to the Dixie Shores Missionary (334-514-3136) prior to participating in events.

(This is an annual requirement and must be completed only once per club year!)

            Click Here to Download Form -  This form must be signed by the pastor or another church officer and faxed to 334-514-3136, or scanned and emailed to brucep@awana.org, or brought to the Spring Event Training.
